Participant Profile Summary

The participant profile summary allows you to record important information, preferences, routines and support requirements in one place. Once saved, the summary can be viewed by authorised staff through the Care Diary mobile app.

  1. Log in to the Care Diary web portal and select Participants from the left-hand menu.

  2. Search for the participant and select the Edit icon under Actions.

  3. Select Summary from the participant’s menu.

  4. Enter the participant’s important information in the Summary section. This may include:

    • Support preferences and routines

    • Communication needs

    • Important risks or safety information

    • Guidance for introducing new staff

    • Appointment or transport requirements

    • Behaviour support strategies

    • Other information staff should know before providing support

  5. Use the formatting options to organise the information with headings, bullet points, numbered lists or bold text where required. Keep the summary clear, factual and easy for staff to understand.

  6. Review the information carefully and select Save Changes.

  7. Once saved, the summary will appear under the Summary section on the participant’s Participant Details page in the Care Diary mobile app. Staff can review the summary directly from this page. If the full summary is not displayed, select Read more to view the remaining information.

Important: Ensure the information is current, accurate and relevant to the participant’s support. Do not include personal opinions or unnecessary confidential information. Update the summary whenever the participant’s needs, preferences, risks or support arrangements change.
